Insurance Information

OSR accepts most private insurance plans, Medicare, and Worker's Compensation Plans. Our office staff will be happy to contact your insurance company and obtain your current physical therapy benefits and explain them to you.

Our billing department will bill your insurance company directly. We accept cash, check, and Visa and MasterCard for cash pay and patient responsible amounts. Any billing questions can be directed to our billing department by phone or email us here.

Cash Pay: Please contact our office for current cash-pay amounts for physical therapy services.

What to Expect and Bring on Your First Visit

  • Prescription for physical therapy (if you have one)
  • Insurance card(s)
  • Any diagnostic reports including X-ray/MRI reports (if you have them)
  • Dress comfortably with treatment region accessible
  • Initial appointment will last approximately 1 hour
  • Photo ID (your driver's license is the most common form)

Which Insurance Plans are Accepted?

We accept most major insurance coverage (see below). Keep in mind that this is a partial list (major carriers), so if you don't see your insurance carrier that doesn't necessarily mean we don't accept your insurance.

If you don't see your insurance carrier on the list below, call us at our main office at 623.551.9706 (or any of our offices) and we'll be glad to let you know what your coverage is.

At the time of scheduling your first appointment we request that you provide us your insurance information so that we may verify your benefits and coverage.
