Directions to North Valley location from Phoenix
From I-17 and the Carefree Highway exit you'll head East.
You'll pass N. 27th Drive, right after this you'll see Mountainside Fitness on the south side of the street - OSR Physical Therapy is located inside Mountainside Fitness.
Directions to Peoria location from Phoenix
Take the 101 to Bell Road.
Head WEST on Bell to N. 99th Ave., Turn RIGHT and head NORTH (99th Ave turns into N. Lake Pleasant). When Lake Pleasant deadends turn LEFT and head NORTH.
OSR Physical Therapy is on the NW corner of Yearling and Lake Pleasant.
Directions to Gilbert location from Phoenix
Take the 101 or 51 past US 60 to the Loop 202
Head WEST on the 202 to Val Vista Drive. When exiting on Val Vista turn LEFT and head NORTH.
OSR Physical Therapy is located inside Mountainside Fitness, which will be on your RIGHT, on the EAST side of Val Vista.
