Orthopedic & Sport Rehab (OSR) Physical Therapy was founded in 2002 in Anthem, AZ. Originally located in a retail shopping center, OSR moved to a larger facility in 2004 and is now located in the Anthem Medical Plaza.
		Our modern and spacious 2500 sq. ft. facility includes state-of-the-art rehabilitation equipment and treatment space for out-patient physical therapy needs. Our mission is to provide the highest quality rehabilitation through personalized service to all of our patients. Our facility and staff promote healthy living and return-to-function in a fun and energetic environment that will make your experience in physical therapy memorable.

OSR maintains a staff of highly qualified and trained rehabilitation specialists, support staff, and office staff in order to ensure the highest degree of customer service. Our rehabilitation services are directed by our staff of physical therapists. OSR also maintains staff and associate certified athletic trainers who work not only with physical therapy patients, but local youth
and professional athletes. Support and office staff complete our team and provide help and direction in office management and assistance to our therapists.

OSR is a comprehensive out-patient physical therapy clinic that specializes in treating a wide variety of problems and diagnoses related to: orthopedic injury, sports injury, post-surgical injury, soft-tissue injury, back and neck pain, motor-vehicle accidents, general hand therapy, work-related injury, and neurological disorders. We treat patients from all ages and diagnoses appropriate for physical therapy.
